Case Management - Mental Health

Program Description

Provides services and supports for those who are surviving homelessness in York Region and South Simcoe.
Access to a housing specialist to support individuals in connecting with potential landlords to secure safe and affordable housing.
Access to a Case Manager to work one on one on recovery-focused goals.

Inclusion criteria:
Individuals experiencing chronic homelessness and/or mental health challenges
Individuals and families that are homeless and in need of a rental subsidy to maintain a safe and affordable living environment
Individuals and families in shelters who are in need of connection to mental health services
